Ancestor Charts (Fillable PDFs)
Our society is pleased to offer these printable, fillable ancestry charts to members, provided with permission from the Midwest Genealogy Center (Mid-Continent Public Library) and the National Genealogical Society (NGS). Please use them for your personal research and do not repost or reuse them commercially.

How to download and save the fillable forms (important)
To make sure your typed entries don’t disappear, download the PDF first, then open it from your computer before filling it out.
On a Windows PC (recommended)
- Click a chart link above.
- When the PDF opens in your browser, download/save it:
- Look for the Download button (often a down-arrow), or
- Right-click the link and choose Save link as…
- Choose a folder you can find again (for example: Documents → Genealogy Forms) and click Save.
- Open the saved file from that folder.
- Best results: open in Adobe Acrobat Reader (free).
- Fill it in, then save your work:
- File → Save (to update the same file), or
- File → Save As (to create a new version)
Tip: Consider a consistent filename like:
Surname-GivenName_FamilyGroupSheet_2026-01.pdf
On a phone or tablet
- Tap the chart link.
- Use the Share or Download option and choose Save to Files (iOS) or Download (Android).
- Open the PDF in a PDF app (often Adobe Acrobat) to fill it in.
- Save after editing.
Suggested use
- Use the Pedigree/Ancestor charts to map direct-line ancestors quickly.
- Use the Family Group / Family Unit forms to capture full family structure and keep a running list of sources (crucial for good research).
Special request
Please choose a basic four-generation chart and
submit it to the Ancestor Charts chair who will index it for use by member researchers. The surnames will be published in Family History Capers. We do not collect family group sheets.
These forms are shared for personal research use by our members and include rights restrictions (for example, NGS notes they are for personal use and not for commercial reproduction).
